Admission Guidelines

Pre-Admission
If the patient is not in critical condition, please proceed to the Admitting Office to prepare:
Bring at least one valid government-issued ID.
Prepare personal details: address, phone, next of kin, employer.
Organize health information: medical history, medications, and records.
Bring PhilHealth, insurance cards, and any authorization forms.

Admission Process
Present the ER or Direct Admission Form.
If previously admitted, provide last admission date and attending doctor.
Submit at least one valid ID (or submit later if not available).
The admitting staff will:
Confirm prior records.
Assess financial capacity (no deposit required).
Inform about room options and availability.
Facilitate PhilHealth or HMO assistance.
Patient or relative must:
Fill out the Patient Data Sheet and Data Privacy Consent.
Sign a payment responsibility form and medicine waiver.
The admission kit is given, and they will be directed to the Nursing Unit and PhilHealth Section.
Staff will explain hospital policies and visiting hours.
Patient will be assisted to the Emergency Room or assigned room.

Room Allocation & Transfer
Room assignments are based on patient condition (e.g., private room for infectious cases).
Only Admitting staff can assign or transfer rooms.
The process includes informing the Nurse on Duty, preparing transfer slips (3 copies), updating the system, and physically transferring the patient.

Room Reservation
Rooms may be reserved, but will be released if the patient does not arrive within 24 hours.
Reservations can be canceled in case of emergencies or critical admissions.

Discharge Process
Cut-off for discharge: Weekdays: 4:30 PM | Sundays/Holidays: 4:00 PM
Post physician’s discharge order:
Settle bills at the Cashier (5 discharge slips issued).
Present slips to: Nurse Station, PhilHealth, Cashier, Admitting (with TV remote), and Security.
Orderly assists the patient in exiting the hospital.
